Preparing for Outpatient Surgery

Our goal is to provide you with the best, most effective care so that you can return to your normal activities as quickly as possible. If you have any questions or concerns while you are in our care, we encourage you to ask any member of our team. We will be happy to answer your questions or to assist you.
To get your health information before your procedure, we will either schedule a pre-op visit or phone call. We will call to schedule a time about a week before the procedure, between 9:00 a.m. and 4:00 p.m., to get you pre-registered before your procedure. Please bring your insurance card and photo identification such as your driver's license.
For your convenience, you can download the following information to help you prepare for your procedure or treatment at home. This information applies to all patients who are here for surgery, endoscopy, or pain relief procedures. In addition, we have added some general educational information you may find helpful as you prepare for your procedure.
