Take the Community Health Needs Assessment Survey!

HSHS St. Joseph’s Hospital (Highland) is conducting a community health needs assessment survey among residents within the hospital’s service area in portions of Madison, Clinton and Bond counties. The goal of the survey is to determine the health care needs (physical and emotional) of the area and develop strategies to address those needs. The survey is completely confidential and no identification will be requested. It should take 10 minutes or less to complete. Go here to take the survey now!

SJH appreciates your cooperation in completing this survey as its results will guide HSHS St. Joseph's Hospital (Highland) to develop future programs and services. That's why it’s important that questions are answered honestly and completely. The hospital’s top priority is meeting the health care needs of the community by being the local health care provider of choice as well as providing an extraordinary patient experience. For more information on the hospital's community health needs assessment, please contact Randy Schorfheide, Director of Marketing, at (618) 651-2590.
