For your convenience, we offer sports physicals. Schedule yours with our professionals, but don't wait until too close to the start of the season! Pre-registration is required. For your convenience download the applicable packet (9th grade and below or 10th grade and higher), fill out and send in the paperwork by the scheduled deadline. The highlighted section on the physical form needs to be completed and sent in with your completed pre-registration form.
